LIFE CARE PLANNING

Expert Guidance

A Life Care Plan (LCP) is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs.




The life-care planning expert will review medical records and supporting documentation, conduct a comprehensive interview, consult with appropriate therapeutic team members, prepare preliminary life-care plan opinions, research associated costs and sources, and finalize the report by confirming projects with the client and family, and treatment team members. Included in the final report is an itemized description of the services, the duration of the services (begin/end), how frequently the services will be required (e.g., weekly, monthly), the purpose of the service, and the associated costs per unit and per year.

Our experts are trained to determine an individual’s employability and our reports and expertise can assist in short-term/long-term disability cases. Specifically, we will review medical information, administer vocational assessments, and conduct Transferable Skills Analysis and use other vocational tools to determine if an individual with an impairment is capable of working.
